Job Description

About Us: We are seeking for our client a compassionate, autism-aware, and open-minded individual to be a Life Skills Coach and Case Manager for their 23-year-old trans autistic daughter located in Charlottetown, PEI. This role is instrumental in supporting her journey toward greater independence, confidence, and overall well-being.

Job Summary: As a Life Skills Coach and Case Manager for their daughter, your primary responsibility is to facilitate her growth and development. This multifaceted role requires a dynamic individual passionate about positively impacting their daughter’s life through continuous learning, planning, and effective collaboration. As her Life Skills Coach, you will employ a curious and creative approach to tailor educational strategies to her unique learning style and work with her one-on-one. You will assess her developmental needs in this role and establish SMART goals across key areas. Your responsibilities extend to delivering education through direct teaching, coaching, and collaboration with other professionals. Documentation and planning are integral components of the position. You will maintain a progress dashboard for her SMART goals, adapt them as she progresses, and contribute to long-term vision planning. As her Case Manager, you will be the hub of a network of support that includes her family, her roommates, and professionals. Coordination is crucial, as you'll identify and manage resources such as occupational therapy and personal training to assist you in helping her achieve her goals. You will also be the point person in her life, helping her navigate her daily challenges and opportunities. You will also help her develop systems for coordinating her daily schedule and tools to move her toward greater independence. Leadership skills are essential; you will lead and coordinate her support team through scheduling and regular check-ins. Furthermore, you will hold meetings with key individuals in her life to ensure a cohesive and coordinated approach to her support. You will be responsible for overseeing a small budget dedicated to supplies, tools, and equipment that support her development.

About Their Daughter: Their daughter is a 23-year-old trans-autistic woman living with “supportive roommates” who provide some basic daily living support. She recently left a day program and is now seeking more independence. She has emotional dysregulation challenges and requires daily living support, making steady coaching essential for her growth. She is looking to build social connections, enhance daily living skills, and explore volunteer opportunities.
